Young People Leaving Care

For children and young people with disability who have been in care, life changes drastically when 
they turn 18. At Westhaven, we offer leaving care support services to help make the transition into 
the adult world a little less daunting. 

The National Disability Insurance Scheme (NDIS), disability leaving care services will provide 
supports to assist the participant transition from the responsibility of the Minister for Community 
Services into supported independent living. The service is designed to meet each participants needs
and aspirations, assisting them to live as independently as possible within the community.

At 15, each participant is assigned a Case Manager/Team Leader to commence leaving care 
transition to supported independent living or other options to meet the individuals needs. The NDIA,
FACS and Westhaven staff will take the time to discuss with each particpant their likes and dislikes; 
and what the life they want to lead may look like as an adult.

Participant Information

Westhaven works with individual participants to find employment pathways for young people with 
disability leaving school and planning for the future. Westhaven provides assistance in looking at 
options which consider your disability, housing choices to suit your capabilities and to understand 
your developmental needs.

Examples may include:

  • Developing skills for independent living
  • Economic independence
  • Community connections
  • Accommodation support
Participants with disability leaving care may have more complex support needs due to life events
they have experienced. They may have an intellectual or physical disability, or other disabilities due 
to experiencing separation from their family, placement breakdown, trauma, abuse or loss. 

Westhaven staff identifies these issues and the participants needs and will ensure that Team Leaders
and Support Workers have the correct training to support each participant.
